Damage Claim and Missing Item Form

Please complete this Damage Claim Form so we can assist you as quickly as possible.

This form is required for:

-Shipments that arrived with visible damage

-Orders that are missing parts or products

-Items that were damaged in transit

Before You Begin:

-Inspect your shipment immediately upon delivery.

-For freight deliveries, damage must be reported within 3 days of delivery.

-For concealed damage or missing items, you must report within 3 business days of receiving your order.

What You’ll Need:

-Your order number

-A description of the issue

-Photos of the shipment upon receipt and photos of the damaged item(s), packaging.

📸 Photos are required for all claims to be processed. Please include clear images showing the damage or issue.

Additional Notes:

If you deselected Route Shipping Protection during the checkout process, Fray Fitness is not liable for damage, theft, or loss during transit. Claims without Route coverage are reviewed at our discretion and will require additional costs for replacement.

Once we receive your completed form, our team will review your claim and contact you within 2 business days with next steps.
